the following violations of the hco were found as a result of an on-site inspection by inspector r. dicks on may 1, 2000 and an examination of the hco records at the offices of the department of building inspection on may 5, 2000 by inspector r. bosque for the residential hotel known as the 16th street hotel located at 3161 - 16th street. the following violations are to be corrected immediately: 1. the certificate of use is to be posted in compliance with section 41.6(d) of the hco. 2. the weekly reports were not posted & available at the time of the on-site inspection on may 1, 2000 in violation of sections 41.9(b) & 41.11(c) of the hco. the weekly reports were not properly maintained, none were available nor submitted for review during the examination on may 5, 2000 in violation of section 41.9(b) of the hco. 3. the daily logs were not available at the time of the on-site inspection on may 1, 2000 in violation of sections 41.9(a) & 41.11(c) of the hco. the daily logs submitted during the examination of may 5, 2000 were not properly maintained in violation of section 41.9(a) of the hco. 4. the rent receipts were not available at the time of the on-site inspection on may 1, 2000 in violation of sections 41.9(a) & 41.11(c) of the hco. the rent receipts submitted during the examination of may 5, 2000 were not properly maintained for every required rental payment in violation of section 41.9(a) of the hco. the hotel owner/operator has been instructed to submit the hotel's hco records for the period of may 5, 2000 through july 11, 2000 to inspector r. bosque for further examination on july 12, 2000 at 11:00 a.m. please contact inspector bosque at (415) 558-6202 if you have any questions regarding this notice of violation.
